A partnership of Heartland Foundation, Mosaic Life Care, and North Kansas City Hospital, Healthy Partnerships offers matching grants for collaborative initiatives in northwest Missouri to explore new, innovative ideas that impact critical health issues. Currently, Healthy Partnerships focuses on ideas that support healthy eating and active living (HEAL).
Tax exempt organizations (public entities and 501(c)(3) nonprofit organizations) are eligible to apply on behalf of partnering applicants. Applications must be for include a dollar for dollar match of the total project cost, and applicants must attend a Healthy Partnership Grant Pre-Proposal Briefing. Applications received without briefing attendance will be disqualified.

Examples of funded projects include:
- A school-based program in a small rural community that included a Healthy Heart Challenge combining nutrition education and physical fitness activities
- An accessible tree house and adaptive sports equipment for youth and adults living with physical challenges
- A prevention program that engaged northwest Missouri youth in advocating for tobacco-free environments.
